Does Hormone Therapy Cause Weight Gain? (And Can It Help You Lose It?)

Online Hormone Therapy online with TBTotal.Health Telemedicine

Hormone therapy does not typically cause weight gain, and it isn’t a weight loss treatment either. Most of the weight people gain around menopause or midlife comes from the hormonal and metabolic changes of aging itself, not from the therapy used to treat them. What hormone therapy can do is relieve symptoms like poor sleep, low energy, and mood changes, which often makes healthy habits easier to keep.

If you’ve been told hormone therapy “makes you gain weight,” or hoped it might help you lose some, here’s what the evidence actually supports.

Why does weight creep up in midlife in the first place?

Several things tend to happen at once:

  • Estrogen declines. As estrogen and progesterone drop during perimenopause and menopause, the body tends to store more fat around the midsection rather than the hips and thighs.
  • Muscle mass shrinks with age. Less muscle means a lower resting metabolic rate, so the same eating habits can lead to slow weight gain.
  • Sleep gets worse. Night sweats and sleep disruption affect appetite, energy, and how much you move the next day.
  • Testosterone declines in both sexes. In men, low testosterone is linked with more abdominal fat and less muscle. Women have testosterone too, and it also declines with age.
  • Thyroid function can shift. Suboptimal thyroid function can slow metabolism even when a standard panel looks “normal.”

None of these are caused by hormone therapy. They’re the reason many people seek it out.

Can hormone therapy cause weight gain?

There’s little research showing that hormone replacement therapy is the primary cause of weight gain. That said, the number on the scale can change after starting treatment for other reasons:

  • Temporary fluid retention or bloating can occur in the first weeks with some hormones and usually settles as your dose is fine-tuned.
  • Added muscle can raise scale weight while body composition improves, particularly with testosterone therapy plus strength training.
  • Something else may be going on, such as thyroid changes, stress, or medications.

If you notice weight gain after starting hormone therapy, tell your provider rather than stopping on your own. A dose adjustment or a closer look at your labs often explains it.

Can hormone therapy help you lose weight?

Not directly. Hormone therapy is not FDA-approved for weight loss, and we don’t position it that way at TBTotal.Health. What it can do is address things that make weight management harder:

  • Better sleep and fewer night sweats
  • More steady energy and motivation to exercise
  • Support for muscle mass and recovery
  • Improved mood and focus

Early evidence suggests a combined benefit. A Mayo Clinic study published in Menopause, reported by Axios, found that hormone therapy used alongside GLP-1 medications was associated with about 30% more total body weight loss than GLP-1s alone. That’s an association, not proof that hormone therapy caused the difference, and researchers are still working out why.

Where does a GLP-1 fit in?

For some patients, restoring hormone balance first and adding a GLP-1 a few weeks later works better than starting everything at once. For others, a GLP-1 alone is enough. The right order depends on your labs, symptoms, and goals. Changing one thing at a time also makes it easier to tell what’s actually working. If a GLP-1 hasn’t delivered the results you expected, our post on why a GLP-1 might not be working covers the hormone and thyroid angle in more detail.

Signs hormones may be part of your weight picture

It may be worth a closer look if you’re noticing:

  • Weight gain concentrated around the midsection
  • Fatigue that doesn’t improve with more sleep
  • Hot flashes, night sweats, or disrupted sleep
  • Difficulty building or keeping muscle
  • A stalled scale despite consistent effort

Questions to ask your provider

  1. Which hormones are actually low, based on my labs and symptoms?
  2. Could thyroid function be playing a role?
  3. What changes should I expect in my first few weeks, including scale changes?
  4. How will we track whether this is working, beyond the number on the scale?

FAQs

Does estrogen therapy make you gain weight?

Physiologic dosing does not typically cause weight gain. Some people notice temporary bloating early on. Persistent changes are worth discussing with your provider.

Will testosterone therapy make me gain weight?

It can raise scale weight if you’re building muscle, even as body composition improves. It isn’t a weight loss treatment on its own.

Is hormone therapy a replacement for a GLP-1?

No. They do different things. A GLP-1 primarily regulates appetite and glucose signaling, while hormone therapy addresses hormone deficiencies. Some patients use both under provider guidance.

How long before I notice changes in energy and sleep?

Many patients notice sleep and energy changes within a few weeks, with body composition changes taking a few months. Timelines vary.

Do I need labs first?

Baseline labs are typically recommended so treatment is based on your actual levels, not symptoms alone.

The bottom line

Hormone therapy isn’t the cause of midlife weight gain, and it isn’t a shortcut to weight loss. It’s a way to treat real hormone deficiencies, which can make everything else, sleep, energy, training, and appetite, easier to manage. If weight is part of what’s bothering you, start with a proper evaluation of your hormones and thyroid.

Hormone therapy and weight loss medications should only be started, adjusted, or stopped under medical supervision. Individual results vary. Some hormone medications are FDA-approved; compounded bioidentical formulations may be prescribed when an FDA-approved option is not available or clinically appropriate, and are prepared by licensed pharmacies for individual patients.
